We need this from you
- Bring all of the eyeglasses you currently use, including any over-the-counter reading glasses.
- If you wear contact lenses, bring the boxes, labels or individual lens packages if possible.
- If you have a copy of your previous eyeglass and/or contact lens prescription, it is helpful to bring it.
- You may print a copy of our New Patient Medical Form, complete it prior to your appointment, then bring the completed form to your appointment.

Like many doctors’ offices, we need your contact information, medical history, insurance coverage and a HIPAA form for our records. For your convenience, we have made the patient forms available online so that you can print and fill them out at your convenience. When you come in for your appointment, please bring the completed forms with you and provide them to the receptionist when you arrive. If you prefer, you may arrive for your appointment a few minutes early and complete the forms in our office.
